Talking about Your Wishes: Five Wishes

This workshop will focus on "Five Wishes," America's most popular advance directive. "Five Wishes" offers a "user-friendly" way for all adults to explore their choices and express their preferences to family and doctors before a health crisis. The booklet uses everyday language to cover important medical and legal issues, but it also addresses personal and spiritual matters related to comfort and dignity. The workshop will also offer advice on starting the conversation with loved ones and healthcare providers, avoiding family conflict, choosing someone to be your "health care  agent," being a good caregiver, planning for crisis, and taking important steps after "Five Wishes" is completed to make sure wishes are known and followed.
