As patients and families confront the challenges of serious, chronic, or life-limiting illness, they must make a myriad of decisions, evaluating complex medical information in conjunction with their values, goals, responsibilities, and desires. Physicians and other health care professionals can find it difficult to talk to patients and families beyond the clinical facts - and yet such conversations are vitally important to determine treatment plans and provide high-quality care. This session will dismantle the barriers to meaningful conversations with patients/families and provide workable tools for healthcare professionals to facilitate advance care planning conversations.
